Yes but all ways in pak128 have been drawn with new offsets. These new ways will be required to use double heights - so to maintain compatibility for older addons a fix of some kind is required.

Yes this extension request is all just for the benefit of maintaining compatibility with closed source addons but this is something we try to do where ever possible.

Between trains and tracks there is exactly a 4 px gap: as you can see, nothing earthshaking.
(Basically new railtracks alignment should more or less match Pak128.Japan's)

Indeed, 4px isn't much visually.  It's there, you can notice, but it's not that big of a deal.

Just because my last suggestion seems to be ignored:
Use the old offset for all vehicles even in the new pak128 rails: But there will be a setting in simutrans to only shift rails vehicles 4 pixel down. Just keep the old alignment for every rail car but use new rails, new stations and the like. Eternal vehcile compatibility (also with pak128.britain guaranteed).
