No new industry chain after starting the map for 11 (in-game) years

I've started the map at January 1928. The map started as 128x128, but then I expanded the map once to 192x192 with 10 new towns. However, there's still not a single new industry spawning as of 1939. The bits_per_month is set to 19, but it's simply unreasonable to not have a single new industry after 11 years in this game. I've tried playing on a map with bits_per_month set to 18 (though the map is 256x256 from start) and new industries started spawning after 10 years. Is the small initial map size to blame? After 11 years, the only consumer of the oil refinery is the gas station...

bits_per_month is not responsible for creating industries, but rather industry_increase_every. The key point is population growth.

Quote# when a city reaches 2^n times of this number
 # then a factory is extended, or a new factory chain is spawned
 # industry_increase_every = 2000

But does the presence of existing industries affect the creation of industries? I've heard that if the map has no industries at all, then no new industries will ever spawn. Also, does expanding the map affect industry growth other than allowing industries to spawn in the new areas?

I see. The default simuconf.tab for pak128 has industry_increase_every 5000. Despite this, there should be a new industry spawning when Liverpool hits the 10000 population mark. I've make a save before I built anything in 1928 and the population was only 5428, and after 11 years it nearly tripled. Is the poor industry supply chain to blame? Or does expanding the map affect industry spawning? Thanks.

10000 is not a trigger point with industry_increase_every=5000.  2^n*5000 -> 5000, 20000, 40000, ...
Population growth will be smaller if you don't deliver all the goods being produced.  Same if you don't deliver all passengers and mail. Population not growing above trigger points - no new industry.
